Specialist Clinical Pharmacist – Older Peoples Service

We require a pharmacist with excellent clinical knowledge and interpersonal skills to join the pharmacy team at the Royal Alexandra Hospital (RAH). This is a full time clinical post with the opportunity to gain experience in Older People and Stroke Services. You will provide specialist clinical pharmacy support to the Older Peoples wards at RAH. These consist of a 12 bedded Older Adults Acute Assessment Unit, a 30-bedded Stroke ward and 90 other acute beds split across 3 wards. You will liaise with colleagues and be responsible for prioritising the pharmaceutical care of all in-patients and the clinical screening of all discharge prescriptions within the Older People wards using the triage and referral model. Assistance with local non-formulary and unlicensed medicine requests may be required. Working within the RAH provides a unique opportunity to become a key member of the MDT and ensure patients receive the best possible pharmaceutical care. This post will have the opportunity to develop and strengthen links with the interface team to ensure good continuity of care for our those who require Older Peoples services. You will be a registered pharmacist with GPhC and have significant relevant post registration experience The Pharmacy department supports the Stage 2 Vocational Training Scheme for hospital pharmacists; SVTS mentor training would be encouraged. You would be required to participate in on-call, weekend and bank holiday rotas.
