What should be included in the medical report for PwD?

• corresponding code of the International Classification of Diseases (ICD); It is important that the CID refers to the sequel – not the cause of the disability. Hearing deficiency

Those who have hearing loss must attach the copy of the audiometry, which is the exam responsible for verifying the existence of the limitation, to the medical report. This examination must indicate whether the deficiency is bilateral, partial or total. To be entitled to quotas, the hearing loss must be 41 dB or more, measured by audiogram in the frequencies of 500Hz, 1,000Hz, 2,000Hz and 3,000Hz; how many decibels.

Visual impairment

Visually impaired people must attach a report by an ophthalmologist to prove their disability. To be entitled to the quota law, visual acuity must be equal to or less than 0.05 in the best eye, with the best optical correction; or that visual acuity is between 0.3 and 0.5 in the best eye, with the best optical correction; or fit in cases where the sum of the measurement of the visual field in both eyes is equal to or less than 60º; or having any of the above conditions occur simultaneously.

Mental disability

Those with mental disabilities must also attach a report signed by a psychiatrist and / or psychologist that specifies their condition very clearly. To be entitled to the quota law, it is necessary to prove that you have significantly lower than average intellectual functioning, with manifestation before the age of 18 and limitations associated with two or more areas of adaptive skills (communication, personal care, social skills, use of the community , use of community resources, health and safety, academic skills, leisure or work).

Physical disability

It is necessary that the report proves complete or partial alteration of one or more body segments with impairment of physical function, presenting itself in the form of paraplegia, paraparesis, monoplegia, monoparesis, tetraplegia, tetraparesis, triplegia, triparesia, hemiplegia, hemiparesis, ostomy, amputation or absence of limb, cerebral palsy, dwarfism, limbs with congenital or acquired deformity, with the exception of aesthetic deformities and those that do not cause difficulties in performing functions.
