Community Staff Nurse – Primary Care and Community Services

Working within the Primary Health Care team and under the direction of the District Nurse you will assess and deliver an agreed plan of care to individuals within a community setting and the opportunity to develop and enhance clinical and managerial skills, providing support for the District Nurse, within the community environment. You will be expected to provide an accessible, appropriate, high quality, culturally sensitive and effective nursing service to all age groups in the community, by working in partnership with service users, care providers and other agencies. You will support the District Nurse/Charge Nurse in delivering care to patients on their caseload and will be responsible for providing skilled nursing interventions for patients within the caseload. It is essential that you are currently a Registered Nurse with the NMC and that you community nursing experience. It is essential that you have a current driving licence. The shift pattern for this post is worked over 3 days per week including evenings and 1 weekend in 4. As holding current full professional registration is an essential requirement for the post, candidates who do not complete the professional registration section of the application form or who do not hold current full registration may not be considered for shortlisting.
