Wherever you are, you will be central to our assessment and care management services with a lead responsibility for the provision and promotion of independent living, consistent with National, Council and Service policies, procedures and practice. You will contribute to the development and implementation of relevant statutory plans, including Service policies and procedures, and liaise with other services, agencies, service users and carers to contribute to the identification of need. You will also undertake assessment and planning with people affected by/living with ill health, disability, long term conditions and general ageing to provide profession specific information, advice and interventions. Possession of a BSc in Occupational Therapy or Diploma in Occupational Therapy and registration with the Health and Care Professions Council are essential. In addition, you must have experience of care planning for individuals’ and managing a case load in a range of settings working with people with disabilities and experience of multi disciplinary/multi agency working. A sound knowledge of medical conditions and understanding of the social model of disability and ability to maintain a professional manner at all times and demonstrate anti discriminatory practice are also essential. You will require to become a Protecting Vulnerable Groups (PVG) Scheme member prior to commencing in this role. Temporary posts are temporary for three months and may be subject to review.
