Care Co-Ordinator

Advert Care Co-ordinator (13/1446M) (Fixed Term) Dumfries £23,331 – £25,116 This is a time of significant change for both the Social Work Service in Dumfries & Galloway, and the wider health and social care sector in general. This post offers real opportunities to contribute to service delivery and development within both single and multi-agency settings. You will work 36 hours per week as an integral member of a busy adult services social work team, working mainly with older adults and their Carers/families. Your primary responsibility will be to work in partnership with the person to assess their need for care and support and to help formulate and review outcome based plans. This will be in conjunction with Carers and families where appropriate, and will also involve working in partnership with colleagues from health, police and third sector. You will need to be able to work under your own initiative and prioritise your work accordingly. You will also have knowledge of the major themes which are relevant to health and social work services, and an awareness of the functional impact of old age, ill health and disability. You will be expected to practice in accordance with legislation, in particular the Social Care (Self Directed Support) (Scotland) Act 2013 and the Adult Support and Protection (Scotland) Act 2007. You must either hold or be working towards an SVQ Level 3 or equivalent qualification in health and social care, and experience of working in a health or social care setting is also an essential requirement. You must have excellent communication skills, both verbal and written, and a good working knowledge of the Mosaic system is desirable. You will receive regular supervision and support from the Senior Social Worker and training opportunities as they arise, in addition to ongoing support from team colleagues. A current driving licence is an essential requirement of this post. However, if you have a disability which precludes you from holding a driving licence consideration will be given if alternative transport can be arranged. This is fixed term for up to a year to cover maternity leave. This post may be suitable for secondment. Agreement from your line manager must be sought before applying for this post. This post is considered as Regulated Work with Vulnerable Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. From 28 February 2011, preferred candidates will be required to join the PVG Scheme or undergo a PVG scheme update check prior to formal offer of employment being made by Dumfries and Galloway Council. Applicants who have been resident outside the UK within the last 5 years, if offered a position, in addition to a Disclosure Scotland Check, will be required to provide a completed police record check from the police authority in the country or countries in which they were resident.
